By using services provided by Erle Jaeger YOU ARE INDICATING YOUR UNDERSTANDING, AGREEMENT and CONSENT TO THE FOLLOWING TERMS AND CONDITIONS: A) Our services are limited to adults. By soliciting and accepting our services you certify that you are an adult (18 years or older). If you are not an adult, your parent or guardian must consent in advance to the use of our services. B) You are consenting to participation in an assessment, consultation and /or treatment to review your problems, discuss the course and time line of your symptoms, review any possible risks to you, and explore your relevant history. You consent to working with your therapist to develop and implement a treatment plan aimed at helping reduce your symptoms and improve your ability to function. Treatment my include coordination with other relevant service providers. C) You may at any time, refuse to undergo any particular treatment, or accept any recommendations for treatment, and you may withdraw your consent for services at any time by simply indicating this to your therapist. D) That whatever your location, any treatment provided by Erle Jaeger will be considered to have been undertaken in the Province of BC and that the Courts of BC shall be the only ones that have jurisdiction to entertain any complaint, claim or cause of action, should you decide to commence any such legal proceedings against Erle Jaeger. OTHER TERMS AND FREQUENTLY ASKED QUESTIONS  We are committed to maintaining client privacy and refraining from inappropriately using information about our clients and website visitors. All personal and professional counseling communications between the Erle Jaeger and those using our services are treated as privileged and confidential communication. However, there are limited exceptions to this policy as required by law.  If it appears to your therapist, or if you advise your therapist, that you are about to do physical harm to yourself or others or, that you are engaged in "child abuse" or "elder abuse" as defined in and under local or provincial laws, the protections afforded by "privileged and confidential communications" do not apply and Erle Jaeger is bound by specific reporting requirements and ethical principles.  There may also be limited legal circumstances where Erle Jaeger is required to disclose information to third parties, (e.g. where a law, regulation, search warrant, subpoena, or court order requires us to do so).  We collect individual personal information that you voluntarily provide while utilizing our services. This information typically includes your name, address, phone number and other contact information.  We also collect information that you voluntarily provide through your responses to assessment forms and surveys. You may also provide individual information to us by other e-mail communications.  This information is treated as privileged and confidential information and under no circumstances is it passed on to third parties except where the law specifically requires us to do so under the specific conditions mentioned above.  We will not disclose to any unaffiliated company any individually identifiable information that you provide to us, except under the limited circumstances described under the law and this policy.  Payment is due before commencing each session and fees can be paid directly through interact transfer from your bank account. In the event of delinquent payment for services. Erle Jaeger reserves the right to use a collection agency to ensure payment.  When you provide assessment information to us for therapeutic purposes, we will not disclose that information, without your written consent. We reserve the right to have an independent supervising therapist monitor clinical treatment for the purposes of supervision and quality control, however, such communications otherwise remain privileged and confidential.  If your participation in our services is covered by you employee assistance plan, or insurance policy, we may provide general demographic information about the overall use of our program to covering insurance companies - however, no personal information is ever shared with insurance providers, website services, other mental health professionals, that would allow you to be individually identified or contacted. We want your individually identifiable information to remain as secure as reasonably possible. To this end we utilize data security measures to assist us in protecting your information.  E-mail to Erle Jaeger is processed as regular e-mail. It is up to you to ensure your privacy as an end user. For example, we can assume no responsibility for who may read the e- mail messages you receive from us once you receive them. Telephone sessions and live chat sessions used for consultation, are as secure as we can make them as we have no control over the telephone extensions at your location or the servers used for Zoom.
